I'm using a T-Flight Hotas X which is similar, but slightly different from the stick in the picture above.[41i3vQDSjDL]
I use Xpadder to map some of the controls.  I use the throttle for impulse and the four thumb buttons on the front of the throttle to select Warp 1-4.  I use the first index finger button on the back of the throttle to select reverse.  On the stick; I use Left-Right Hatswitch to move back and forth between consoles and Up-Down Hatswitch to Zoom In-Out.  I use X-axis to steer left and right and Y-axis to dive-climb, BUT, I set a large deadzone on the Y-axis so the dive-climb has to be intentional and not as a result of accidental input while turning on the X-axis.  The trigger is request dock and thumb is shield toggle.  It works pretty well for me.  My main desire is that Reverse did not need to be toggled and could be on the back half of the throttle axis.  I might be able to create a macro in Xpadder to accomplish that, but it hasn't been a priority.

How did you manage to do that? Right now my Thrustmaster Tflight Hotas X's thottle is not working. I tried to use UI_INPUT_JOY1_AXIS4 for it, but it does not seem to detect my throttle input. All my other axis are working, just not the throttle...

Did you have to use an external program? My other axis are working fine without one...

What Dave said. We've been using JoyToKey and the config Dave linked to.
We both use the same setup since we run our bridges together at cons and it helps keep things consistent for players.

This year, I built a simple base that splits the HOTAS and holds the throttle and stick close on either side of the Helm laptop. I'll post pics in a day or two.
